Confidential Policy

Welcome to Mind.Strong.Live.Free! We are committed to ensuring the privacy and confidentiality of the information you share with us. This confidentiality policy outlines the measures we take to protect your personal information.

Client Privacy and Confidentiality:

  • All information shared during sessions, whether in person or online, is kept strictly confidential.
  • Information is protected in accordance with Canada’s Personal Health Information Protection Act, 2004 (PHIPA).
  • Your information is used for administrative purposes and maintained electronically using PHIPA-compliant electronic health record keeping software.
  • Unless required by law, your file information will not be released without your written permission.

Limits to Confidentiality

The only thing that takes precedence over confidentiality is safety. There are four limits to confidentiality where your counsellor is legally (and ethically) obligated to report to the appropriate authorities:

1. Intent to harm yourself or others.

2. If there is risk of harm to a child and/or dependent.

3. If you disclose abuse caused at the hands of a licensed professional (i.e. a doctor or counsellor).

4. If your therapist’s case notes are subpoenaed for court.

Appointment Information:

Appointments are available between 9am and 5 pm, with extended hours on Tuesdays and Thursdays. Sessions are 50 minutes in length, with the last 10 minutes utilized for reflection and reporting.   • Our practices are currently online, and we use the secure and confidential Jane platform for virtual sessions.

Fee Structure:

The fee is $145.00, inclusive of all taxes, payable at the time of service.

Cancellation and Rescheduling Policy:

Provide 24 hours’ notice for appointment changes, or as soon as possible in emergencies. Multiple late cancellations and/or no-shows may result in billing the full session fee before rescheduling will be permitted.

Consent Forms:

  • You have the right to review your progress, withdraw consent, end the counselling relationship, and access your information.
  • Confidentiality exceptions include danger to self or others, child abuse reporting, and reporting sexual harassment by a professional. Your concerns about therapy can be addressed with Shani Ellis-Alman or submitted to the Ontario College of Social Workers and Social Service Workers.

Terms of Use:

  • Therapy treatment is consented to at a rate of $145 per session, payable at the time of service.
  • Sessions are approximately 50 minutes long, and 24 hours’ notice is required for cancellations.
  • If concerns arise, you are encouraged to discuss them with your therapist, and if necessary, further action can be taken.
